Mycophenolate mofetil (MMF) was initially introduced without the need for therapeutic drug monitoring (TDM), using a fixed dose of 2 g/day. However, clinical experience showed wide inter- and intra-patient variability due to its complex pharmacokinetics. Factors such as kidney and liver function, serum albumin, absorption changes, and interactions with other immunosuppressants can significantly affect mycophenolic acid (MPA) exposure, making TDM valuable for optimizing efficacy and safety.
