To approach a patient with a thyroid mass, start with a detailed history and physical exam, followed by a TSH blood test and a thyroid ultrasound to characterize the nodule. If the nodule has suspicious features on ultrasound (e.g., solid, hypoechoic, microcalcifications, larger than 1 cm) or the patient has a history of risk factors for cancer, a fine-needle aspiration (FNA) biopsy is recommended for diagnosis. The final treatment, which may include surgery, medication, or active surveillance, is then determined by the FNA results and nodule characteristics.
