A stoma history involves understanding the patient's condition (IBD, cancer), type of stoma (ileo, colo, urostomy), surgical history, diet, medications, and psychosocial impact, while examination focuses on inspection (color, size, shape, protrusion, surrounding skin, output) and palpation (tenderness, hernias, cough impulse) to assess stoma health and identify complications like prolapse, retraction, bleeding, or skin breakdown, ensuring proper functioning and comfort
